If you or a family member have diabetes and are admitted to OSF St. Joseph Medical Center, a certified diabetes educator (CDE) is available to help provide diabetes care and education.
Whether it is help with managing your diabetes or assistance with obtaining diabetic supplies, our certified diabetes educator can provide support unique to your needs to control your diabetes. Inpatient diabetes care and education can serve as a bridge to ongoing outpatient diabetes management and diabetes education.
Diabetes is a complex condition requiring ongoing care management in an outpatient setting. Upon discharge, a referral can be made to OSF Medical Group - Endocrinology & Diabetes for diabetes management and/or diabetes education. To be seen, a referral is needed by your primary care provider.
OSF Endocrinology and Diabetes staffs a board certified endocrinologist and a nurse practitioner/certified diabetes educator who can both see patients for diabetes management. The team also includes registered dietitians and registered nurses. Many of the team who provide nutritional counseling and diabetes education are also certified diabetes educators.
OSF Endocrinology and Diabetes also offers free pre-diabetes classes. Additionally, nutritional counseling is available for diabetes, weight management, high cholesterol, gastrointestinal conditions and other various conditions.
