29.1 million Americans have diabetes. At OSF HealthCare Saint James - John W. Albrecht Medical Center, our Diabetes Care program provides education and support to both patients in the hospital (inpatients) and patients not hospitalized (outpatients).
Our program is recognized by the American Diabetes Association, which assures the program follows the National Standards for Diabetes Self-Management Education and Support.
Team members visit patients who are newly diagnosed or who have special diabetes management issues.
Food and nutrition issues are the focus of our dieticians, while our nurse educators provide important education and resources about your diabetes.
Working to help patients understand this disease and how to manage it, our team will also work closely with physicians to monitor your glucose levels while you are hospitalized.
Our educators provide outpatient education, and work with medical providers to start patients on insulin pumps or perform continuous glucose monitoring.
Our education team is available Monday through Friday, 8:30 a.m. - 4:30 p.m.
Patients are seen by appointment only; physician referrals are required.
