OSF Holy Family Medical Center is a 23-bed acute/intermediate care, critical access hospital. We are located in the rural farming community of Monmouth, Illinois and currently serve the patients and families of Warren, Henderson, and Mercer counties.
OSF Holy Family provides a broad range of acute care and outpatient services including a variety of specialist, emergency, rehabilitation, and diagnostic imaging services.
OSF Holy Family Medical Center is a tobacco-free and smoke-free campus. OSF HealthCare is committed to the promotion of health through the treatment and prevention of disease, and for providing a safe and healthy environment for our employees, physicians, volunteers, visitors and those we serve.
Consistent with this commitment, OSF HealthCare recognizes the adverse health effects of tobacco products and second-hand tobacco smoke.

OSF Medical Group - Monmouth, "a department of Holy Family Medical Center," is located on the OSF Holy Family Medical Center campus. It provides primary medical care for patients of all ages.
OSF Medical Group - Monmouth is staffed by several physicians, physician assistants, advanced practice nurses and support staff.
A variety of different services are provided through the Specialty Clinic including Cardiology, Diabetes Care, Podiatry, General Surgery, Pulmonology, Sleep Lab, Urology and Women's Health.
OSF Specialty Clinics-Monmouth is located in the Rehabilitation and Specialty Clinics wing on the north side of OSF Holy Family Medical Center.
