Getting older doesn’t mean you have to give up your freedom. If you need health care support but want to live in your own home, there’s a choice for you.

OSF PACE stands for Program of All-Inclusive Care for the Elderly. It’s for people who need help with their health and wellness in order to stay independent. OSF PACE gives you your own personal team who can help with your daily needs.

As a participant, we assist you with things like managing medications, preparing meals, bathing and more. Plus, this added support helps lighten the load for caregivers and loved ones.

Other benefits include:

  • Support from health care providers: You have easy access to doctors and nurses. Your providers know you and specialize in treating seniors. OSF PACE even gives you free rides to your appointments.

Services

OSF PACE takes care of all your health care needs, including:

  • 24-hour access to our care team
  • Adult day services (up to five days a week)
  • Emergency services
  • Outpatient care
  • Hearing care
  • Home care, including home care aides, personal care help and chore services
  • Inpatient hospital and nursing home care
  • Laboratory tests, X-rays and other diagnostic procedures
  • Medical and non-medical transportation services
  • Medical supplies and appliances
  • Nutrition support and special diet assistance
  • Physical, occupational, speech and respiratory therapies
  • Prescription drugs
  • Psychiatric care
  • Respite care
  • Routine preventive care (dentist, eye doctor, podiatrist, etc.)
  • Palliative care and end-of-life services

OSF PACE ServicesCan I enroll?

If you’re interested in enrolling in OSF PACE, the first step is to find out if you’re eligible.

In order to qualify for OSF PACE, you must:

  • Be age 55 or older
  • Meet the Illinois requirements to live in a nursing home
  • Be able to live safely at home
    • OSF PACE staff will assess if you can be safely cared for in your home.

Whether you're an active participant or exploring if OSF PACE is right for you or a loved one, below are answers to some of the most common questions we receive.

What is the cost?

OSF PACE services are paid for by Medicare, Medicaid, private insurance or by a monthly premium paid by the participant. See the OSF PACE Enrollment Agreement for more details.

Will I still have Medicare or Medicaid coverage?

When you enroll in PACE, you will no longer be enrolled in other Medicare and Medicaid programs. However, you won’t lose any coverage you currently enjoy. Instead, you would keep your current benefits while also gaining the benefits provided by OSF PACE.

If you decide to join a Medicare plan after enrolling in OSF PACE, this would result in your voluntary disenrollment from OSF PACE. More details can be found in the Enrollment Agreement.

Can I continue to see my preferred doctors?

You must select providers from the OSF PACE network. However, there are exceptions in rare cases. OSF PACE has contracted with certain medical providers (doctors, hospitals, etc.) to deliver covered services, so it’s important for participants to know which providers are in the OSF PACE network.

Where can I find information about things like filing a grievance or appeal, appointing a representative to make decisions on my behalf or disenrolling from the OSF PACE program?
