Skills Validations

EMT, EMT-Intermediates, Paramedics and Pre-Hospital RN’s, are required to attend and successfully complete a Skills Validation session two times per licensing period. Annual Agency Continuing Education Application

This application is designed to support our System Agencies by providing them with Continuing Education templates to follow as each provider conducts their own “in-house” training. Those requesting assistance in the delivery of their Continuing Education should contact their OSF Northern Region EMS Staff Representative. Please read the cover page and instructions carefully before completing this application. EMS providers can attend a Continuing Education offering that is level-appropriate at any System Agency. Please contact that System Agency directly to confirm the date and time, etc.

Use of this template is suggested, but not mandatory.

EMS agencies not using this template should submit all of the same information as outlined in the CE Application Cover Page and Instructions, plus three (3) level-specific objectives for each topic. Without this information, we cannot process your application, and it will be returned to you for completion.

Please remember – you can only offer CE up to your agencies level of licensure.
