Why you should join our Patient and Family Advisory Council

One of the ways OSF Home Care Services partners with patients, family members, and caregivers is through our Patient and Family Advisory Council. If you or a loved one has experienced care through OSF Home Care Services, we need your help. Services may have included:

  • Home Health (nursing & therapy)
  • Home Infusion Pharmacy 
  • Home Medical Equipment 
  • Hospice & Hospice Home
  • Outpatient Palliative Care
  • Skilled Nursing Facility Practice

How you can help

You will attend monthly meetings to collaborate with our clinical and non-clinical team members to provide guidance on how we can improve our patient and family experience. Your unique perspective and valuable feedback about the care you or a loved one received will help us improve for others. 

If you would like to share your ideas to help improve care for patients and their families, we invite you to join our team as a patient advisor. We have councils located in:
