The Centers for Medicare & Medicaid Services require a face-to-face visit and documentation before home health services can begin. This documentation must include the encounter date (within 90 days), qualifying diagnosis and qualifying home-bound status. The order must be signed by a PECOS-enrolled physician, not an advanced practice provider (APN/PA) or resident.
The Centers for Medicare & Medicaid Services (CMS) require face-to-face encounters for durable medical equipment. The encounter must occur within the 6 months before the order is written for the DME.

*Note: All referrals to our hospice program are immediately elevated to our local hospice manager in order to get your patient into hospice care as soon as possible.

Epic physicians can submit a referral electronically using the Home Health or Hospice Referral codes, 136 and 137, in Epic.
We have a home care coordinator at each OSF hospital and medical center who can meet with patients to discuss their care options. To set this up, please call us at (800) 673-5288.
Anyone can make a referral to OSF Home Care. You can even call to set up your own home care service. Our team can set up an evaluation visit to determine if you or your loved one needs home care services.
However, a physician’s order is needed to start services. If the OSF Home Care team determines a need for these services, we will contact your physician to obtain the necessary order.
