Appointments are usually made by physician referral only. On the day of your appointment, please arrive 20 to 30 minutes early. Wear loose-fitting clothing and footwear that is easy to remove. Expect your first visit to last an hour to an hour and a half.  Follow up visits typically last 30-45 minutes. Please bring the following items with you:

• Health insurance information, including authorizations or referrals
• Completed patient information form
• List of prescription medications and dosages, along with the pill bottles
• List of allergies, including medications
• List of questions you may have
• Recent test results related to your condition, including any radiology films or discs
